I have helped a few children who needed shoes to accommodate their feet with six toes, and I believe I can help your child too. Below you can find some examples of children with six toes:

You must avoid shoes that come with pointy toe-boxes. Shoes that are too narrow for a child with six toes can lead to several foot issues such as blisters, calluses, ingrown toenails, and corns, just to name a few.

The process of fitting them in a pair of shoes is not that much different than that of a child who has five toes, but we must consider the extra room that we need in the shoes for that extra toe. We need to prevent those toes from getting cramped inside the shoes or overlap with the other toes.

What Makes the Shoes that I Recommend Ideal for Children with Six Toes?

There are two important features that the shoes must provide to prevent this from happening. The shoes must provide extra depth and come with round toe-boxes.

This is my go-to shoe brand for children who have really wide feet since it tends to fit the widest feet. Remember that your child’s extra toe is taking up extra space inside the shoe.

Unsure About What Shoe Size You Should Order?

In each of the descriptions of the shoes, I indicate whether you need to order a half size or a whole size bigger than your child’s current foot size to provide the correct amount of growing room.

*A half a size bigger translates to 3/16 of an inch or 0.4 centimeters of space between your child’s longest toe and the end of the shoes.

*A whole size bigger translates to 3/8 of an inch or 0.8 centimeters of space between your child’s longest toe and the end of the shoes.

      • Hi Juan Pablo, I really could use your help. My son has Polysyndactoly of the big toe on one foot (so two big toes fused together) and a regular foot on the other. His foot with the condition is around 2 inches across and his foot is 4 inches long. His other foot is about a centimeter/ a centimeter and a half smaller width wise. However, he is only 11 months old and he gets surgery right after his second birthday in 2024. Please let me know if there is anything you need from me to help!

          I hope you and your family are well. I don’t have polydactyly but I can definitely help your son find the correct pair of shoes. The key is to find shoes with extra depth and rounder toe-boxes, and most likely they will have to be labeled as extra wide (XW).

          • Thank you for your response. I can send you pictures but he has extra extra wide feet. He has a full metatarsal on each foot. I have tried Teva and each of the options you recommend further up but all are too tight. He currently has to wear teva when going out but we have to limit the time because they look like they are pushing his toe over more ( the end ones , his little toes slightly cross over the other at the tip) I really do appreciate your help.             I really want to help you find shoes that fit your son’s feet and in order to do that taking look at your son’s feet is going to help me determine how wide his feet are and how much room he needs in front of the toes. There is another open toe sandal you can try made by Timberland that fits wider at the front. Just make sure you are ordering the correct size:

      Your article has been so helpful to me! I was born with 6 toes on each foot and so was my daughter. Eventhough mine were removed I still have a difficult time finding ones that for me well. I have been looking all over for shoes to fit my daughter well. The closest that worked for her were stride rite sandals and they were a little too tight. I kept cans of food ontop of them to crush it overnight and that seemed to help. Not the best, but it worked.

      I am going to get the new balance sneakers that you recommended and seemless socks. I need advice with dress shoes or sandals please.

      I always recommend having children fitted for shoes at your local kids’ shoe store that you trust. You need to look for shoes that are deep and come with a round toe-box in order to prevent your granddaughter’s toes from overlapping or rubbing against one another or the side of the shoes.

      If you don’t have a shoe store in your area that you trust, then take a look at some of the best styles that I recommend:

  5. HI, my 14 mo old son was born with an extra little toe on both of his feet but one foot has an extra metatarsal also so one foot is wider than the other. He is measuring a 5.5 XW currently. I have been working with a local shoe store but they are unable to find anymore shoes that will work for him because the front of his foot is so wide now and the heel is narrow. We have tried Naturino’s and Good Soles in the past. I ordered a New Balance tennis shoe I saw you had recommended to try on him but can you recommend a good summer shoe (a croc or something similar) and a dress shoe? Thank you so much!

      Naturino shoes are too narrow and they are also not deep enough. I am not familiar with the shoe brand Good Soles.

      Like I mentioned in the article, the key to fitting a child with six toes is finding a pair of shoes that are labeled as extra wide but that are also deep and with a round toe-box.

      The New Balance shoe styles that I recommend in this article are all deep and come with the roundest toe-boxes, but you should consider providing your child with a pair of shoelaces instead of Velcro closure to save the issue with his narrow heels. A pair of shoes that come with shoelaces will push your kids’ heels back, supporting, protecting, and securing them agains the back of the shoe.

      Crocs might work because they come with such round toe-boxes, but they don’t provide any support or stability. Here are my recommendations:

      Summer Sandal: Option 1 (order size 6)

      Summer Sandal: Option 2 (order size 6)

      Dress Shoe: Option 1 (not sure how dressy the occasion is)

      Dress Shoe: Option 2 (not sure if it will be wide enough but it is one of the widest styles)

  7. Hello, my 9 month old has polydactyly on her right foot & the left is ‘normal’. She’s never worn shoes before. How should I go about looking for the correct size that would be comfortable for both feet. The foot with the extra toe measures at 4 inches heel to toe, 2 inches at the widest point and her instep is little more than an inch. The only different measurement on the left foot is her widest point is 1 3/4 inches.

      You need to look for shoes that provide extra depth and rounder toe-boxes, to allow your daughter’s feet to fit deeply inside the shoes and prevent her toes from rubbing against one another or the side of the shoes.

      Based on the measurements that you sent me (4 inches), your daughter needs a shoe size 3 in order allow for growing room. The issue is that I can’t tell whether your daughter’s feet are narrow, medium, wide, or extra wide based on the other measurements that you sent me.

  8. Hello! My 12 year old son was born with macrodactyl toes in his left foot only. The two impacted toes are huge and require him to need a shoe with an extra wide toe box. Can you help point us in the best place for basketball and football shoes?

      Are you currently living in Sri Lanka? There are a couple of white shoes that provide extra depth and are available in wide and extra wide widths to allow extra space for the extra toe, but I am not sure if those shoes are available in Sri Lanka.
